In consideration of being permitted to participate in the [Event Name] (the “Event”) at [Place of Event]
(the “Premises”), the undersigned participant to the Event acknowledges and agrees as follows:

1. I hereby release [BC Korean Cultural Heritage Society] and its directors, officers, employees, independent contractors, agents, and representatives (together, the “BCKCHS Parties”) from all liability, and I hereby waive as against the BCKCHS Parties all recourses, proceedings, claims, and causes of action of any kind whatsoever, in respect of any and all personal injuries, property losses or any other damages which I may suffer arising out of or connected with my participation of the Event or being in the Premises, not with standing that such injuries, losses or other damages may have been caused by the negligence of the BCKCHS Parties.

2. I hereby acknowledge and agree that:

• While participating in the Event or entering the Premises, I may be exposed to certainrisks and hazards, including, without limitation, slips, trips, falls, equipment malfunctions,theft, and exposure to COVID-19 or other illnesses, and as a result of the aforesaid risksand hazards I may suffer personal injury and property loss;

• I am participating in the Event and entering the Premises entirely at my own risk;

​

• I am solely responsible for exercising due care and taking all reasonable measures to avoid, mitigate, or otherwise prevent the aforementioned risks and hazards;

​

• I will abide by all rules, regulations, policies, and instructions provided by the BCKCHS Parties during my participation in the Event;

​

• I understand that by signing this release and waiver, I will be forever prevented from claiming against the BCKCHS Parties for any property loss or personal injury or other damages which I may sustain while participating in the Event entering the Premises, whether or not such loss, injury or other damages is caused by the negligence of the BCKCHS Parties;

• I understand that BCKCHS Parties would not permit me to participate in the Event or enter the Premises unless I signed this release and waiver; and this release and waiver is binding on myself and my heirs, executors, administrators, and personal representatives.
